Baltimore is disappointing if you considered them a super bowl favorite, but right from opening day it was pretty clear they were not that. They've also been worse than reasonable expectation, but the Redskins have been the single biggest disappointment in football. The surprise with New Orleans and Indy wasn't that they came out of nowhere (like Kansas City), rather that they were known mediocre commodities who made defensive jumps in 2013 that they could have made at any other time, but never did. And I'm not sure how to identify that in a preseason prediction. If you had asked me to go with the worst two defenses in football before the season, I might have said the Saints and the Colts (or Chargers). Actually, the Chargers have been a surprise too, but they don't have a ton of wins to show for it. I may have forgotten the Titans. I thought that team would be terrible. They're...okay.
