She's Brooke's mom who died in the crash JT was involved in. She's also the Angel of Death and some sort of Jesus allegory; notice she was drinking wine and eating bread when Jax bumped into her. When asked who she was, all she responded with is "it's time".

Also if you look at past season's, she makes an appearance in episodes where major characters in Jax's life have died (Tara, Gemma, Opie)
