It shouldn't be a surprise really. People that "abuse" the system do so because the system is so flawed to begin with. People feel disenfranchised and don't have a lot of hope for things to get better. Those with crappy lives really aren't too concerned with eating well or watching their weight. And does it make it any better for someone with money who is obese? They're also going to put a strain on the system with their medical bills.

You are right though, about people with generally crappy lives not really being concerned with a healthy diet. I saw that a lot when I lived in Gainseville, FL. It's so po-dunk down there, and all fried food, sweet tea, budweiser, and Marlboro's... yuck.
